Care at Hospice
We provide round the clock care on our In Patient Unit, where you can be looked after in a home away from home environment.
We provide care and support to our patients in the community and at home - from point of diagnosis to their final days
We have two teams - the Living Well team and our Specialist Palliative Care team - that support people in the community with a life-limiting or life-threatening condition, whether they are in the Hospital, in a care or nursing home, or in their own homes.
Their role is to provide care coordination, advice, and support with any physical, psychological, spiritual or social issues, supporting you and your family. Our aim is to enable people to have the best quality of life.
Listening to your concerns about your illness and those of your family/carer
Giving you the opportunity to discuss your care, what is important to you, and what your wishes are about your future care
Providing information about your illness and treatment
Liaising with other health care professionals and teams involved in your care, be that in the community or hospital, as our involvement aims to compliment care provided by others
Assessing any symptoms that are troublesome and making suggestions of how these can be improved
