Advance Care Planning (ACP) is an important component of the Gold Standards Framework. http://www.goldstandardsframework.org.uk/ ACP is about thinking ahead and is an empowering process for people living with a life-limiting illness, as it gives a sense of control over their ability to live and die well, ensuring their choices and wishes are known.

This evidence based workshop will support participants to build confidence in having conversations with people in your care about what is important to them in their last years of life. The interactive workshop will include an overview of use and guidance on ACP in different settings, ACP for people with dementia, discussion of common challenges and solutions and communication skills. A safe learning environment will be created to provide information and opportunities to consider and reflect on the important role each attendee has to have conversations with their patients/clients/residents regarding their future wishes and preferences.

Sandwiches are available at Jersey Hospice Care at a subsidised rate and can be ordered on the day of the event at Reception.
