Home Care

The Home Care Service is usually the first point of contact patients have with Jersey Hospice Care. Patients are referred to Jersey Hospice Care with the agreement of their General Practitioner (GP).

The Home Care Service consists of a team of specialist nurses who work in the community with cancer and motor neurone disease patients and their families, providing a 24-hour on-call service. The service is entirely confidential and free of charge.

The Home Care Sisters visit patients as often as required to offer their specialist palliative skills, which include advice on symptom control, support for the patient and the family, referral to other agencies and liaison with other professionals involved.

When will I hear from the Home Care Service?

If you have agreed to Jersey Hospice Care’s involvement with your GP, a Home Care Sister will usually make contact with you within three days of receiving the referral.

How do I contact the Home Care Service?

You can contact the Home Care Service at any time by telephoning Clarkson House on 876555. The Home Care Sisters are on duty daily from 8am to 5pm.

Will the same person always visit me? 

A 24-hour on-call service is provided seven days a week. Whenever possible you will be visited by the Home Care Sister appointed to you, however you may meet other members of the team if you require assistance outside normal day-time working hours.  The Home Care Sister will assess your individual needs and from this will develop a mutually-agreed programme of visits, which may be daily, weekly or monthly.  Support will also be given to your family and other carers.
